Evaluate the efficiency and effectiveness of practice outcomes across systems

At our monthly Resident Engagement meeting in February, we had a long and effective discussion about our Household Management Assessment effectiveness such as if the help we give is sustainable, as well as evaluated the ways that we should handle case management, particularly if the team member preforming the HMA visit should handle case management for that resident or if that should go to the regular caseworker on site. We used the social work knowledge and skills of assessment and evaluation as well as collaboration to have a fruitful discussion on this matter, as well as the social work value of competence.  I am going to assist with a questionnaire that will further finalize our collective team decision on that matter. I used the cognitive processes of creating and evaluating and the affective process of valuing.  We used the theoretical framework of the Quality Improvement Theory.
