Competency 7: Assess with individuals, families, groups, organizations, and communities.<\/b><\/span><\/p>\n

Introduction:\u00a0As a professional social worker, I will design and implement comprehensive assessments using a variety of classification systems that assess individuals, families, groups, organizations, and communities. I will gather data such as organizational reviews, client information, etc. using relevant assessments to provide guidance for client interventions, policies, and organizational practices.<\/span><\/p>\n

 <\/p>\n

7.1 – Formulate comprehensive assessments, using a variety of diagnostic classification systems<\/b><\/span><\/p>\n

Course Evidence:<\/strong> \u00a0For SOCW 633 – Psychopathology, Clinical Assessment, and Diagnosis II, I\u00a0developed<\/strong><\/span> a\u00a0Case Construction<\/a><\/span> Paper<\/a><\/span> <\/strong><\/span><\/span>that utilized the DSM-5 and other diagnostic criteria to review a case and create a plan for treatment. I applied knowledge of the DSM-5, treatment modalities, and interpersonal communication<\/strong><\/span> to create a plan for a pseudo-client experiencing trouble concentrating at school. I\u00a0analyzed<\/strong><\/span> her symptoms and used the affective domain of\u00a0receiving<\/span>\u00a0<\/strong>information from the client.\u00a0<\/span><\/span><\/p>\n

Field Evidence: <\/strong>For my practicum, I managed our data evaluation and review processes. Thus, I used our data management system, Client Track, to run reports about data quality and client information. I regularly ran reports and sent updates to staff members about missing data and client information needing changes. I demonstrated the social work value of competence<\/strong><\/span> by choosing to attend multiple trainings and meet one-on-one with the Data Manager at the Chattanooga Regional Homeless Coalition to learn the various diagnostic systems that are used to review the client data. I used the affective domain of organization<\/strong><\/span> to assess the completeness of our digital case files. This allowed case managers the opportunity to update their files to ensure all required information was recorded. I <\/span>integrated this information into our current practices and created a system for reviewing our information and applying<\/span> <\/strong>this knowledge to practice. After running several reports, I sent a Client Track Corrections Email<\/a><\/strong><\/span>\u00a0to my other staff members with a reminder to update their individual client information.\u00a0<\/span><\/p>\n

7.2 – Design and implement organizational and\/or community assessments.<\/b><\/span><\/p>\n

Course Evidence:<\/strong> For SOCW 612, my team developed a mentorship program for children in foster care. As part of our program proposal, we developed a 612 Needs Assessment and Program Proposal Plan Paper<\/a><\/strong><\/span>*(pages 34-38) identifying the strengths, weaknesses, opportunities, and threats for our program. I used my social work skills of written communication<\/strong><\/span> and evaluation<\/strong><\/span> to create this project.<\/span><\/p>\n

Field Evidence:\u00a0<\/strong>For the my program’s COA review, I assessed<\/strong><\/span> if the program met COA policy standards. It came to my attention that our program did not have a clear case closure system for clients who are exiting the program, which was required by COA. I reviewed the COA standards and researched case closure methods for similar programs. I implemented the policy assessment by developing and implementing a<\/span> Case Closure Plan<\/a><\/strong><\/span> with the approval of my supervisor. The plan was adapted from a similar form created by NCCEH. The YHDP plan evaluates the readiness of each client for supporting themselves financially, socially, and educationally. It also provides a grid for helping clients create plans and goals for during and after the program. This document used\u00a0Contingency Theory<\/span>\u00a0<\/strong>by providing opportunities for the case manager and client to adjust their plan according to the evolving needs of the client.<\/span><\/p>\n
