Formulate comprehensive assessments, using a variety of diagnostic classification systems

One day at practicum, I had a resident contact me to say that he thinks that it is time that he move into a nursing home.  I wanted to help to empower him and respect his self-determination, but I was not sure of the protocol for something like that, and at supervision, I discussed it with my field instructor and we came up with the idea to make an assisted living assessment.  I researched to find already existing assessments from insurance companies and other sites, compiled the questions, and used AI to determine the scoring that I should use for it.  The cognitive processes that I used for this was creating and evaluating, and the affective process was responding.  I used a person centered approach as my theoretical framework.
