Assessments are required and important because they provide helpful information about a person’s or group history, diagnosis, former treatments, and scopes of necessary data. Once the assessment is completed, a social worker will analyze the information to develop a treatment plan or coordinate the appropriate services and resources. It is a helpful method in tracking progress, areas of improvement, and documentation.I will utilize various forms of evaluation and measurement tools to engage with people, groups, and communities in order to provide a comprehensive assessment.
7A – Formulate comprehensive assessments, using a variety of diagnostic classification systems
Course Evidence: For the School Social Work Foundation course, I completed a biopsychosocial assessment for a student. I believe biopsychosocial assessments are helpful in gathering information. The skills I used for conducting a biopsychosocial assessment are listening, paraphrasing, and focus. I used two tools as assessment forms. It covered the student’s biological, psychological, and social areas. To view the biopsychosocial assessment, please click here.
Field Evidence: For my practicum, a clinician and I developed a practice framework on screening potential clients for EMDR therapy at the counseling center. We incorporated specific and follow-up questions that can provide information on whether a client meets the necessity of EMDR therapy. (Cognitive level: Application) To view the screening questions for EMDR, please click here.
7B – Design and implement organizational and/or community assessments.
Course Evidence: For my Advanced Administrative Practice Program Development course, my classmates and I created a pseudo program. In valuing service and the importance of human relationships, my classmates and I created an organizational assessment for families with high Adverse Childhood Experiences (ACEs) scores for the Boys and Girls Club. The purpose of the assessment is to evaluate how many families are affected with childhood trauma and provide a program for children and their parents. Field Evidence: I created a needs assessment survey for Collegedale Academy freshman students. The survey results from the students revealed that the majority of freshmen struggle with managing their stress. (Affective level: responding) To view the survey, please click here.
