I created two evaluation forms for the Recovery Kit. The first one is a satisfaction survey form for the DAT responders since they are the ones who will hand out the Recovery Kit to the client. This survey accomplishes two things: it lets the responder know that their opinion and knowledge are wanted and needed, and the client may express an opinion on the kit because the responders have made them comfortable by exhibiting warmth and empathy. The second evaluation form is administered to the clients by the recovery care worker. This form allows the client to give their opinion after a few days removed from the disaster and over the phone, which may help them express their opinion more freely.
